To Complete Registration at Christ Memorial Christian Preschool the following forms must be completed and returned to the preschool director, Heather Gallagher.  If you have any questions regarding this process, please contact Mrs. Gallagher at preschool@cmpcusa.org or 410-997-8011. Form #1: Maryland Health Inventory – A completed Maryland Health Inventory for all children is required BEFORE the first day of class.  This is a state requirement.  The doctor’s examination used to complete the form is to take place within 12 months prior to school opening.  In addition to the Maryland Health Inventory, a separate Maryland Department of Health Blood Lead Testing Certificate is required for all children prior to the first day of class.

If your child may require medication during the school day for a health condition, please submit a Medication Administration Authorization Form completed by a doctor prior to the first day of school.  If your child has allergies that require the use of medication, an Allergy and Anaphylaxis Medication Administration Authorization Plan completed by a doctor must be filed with the school prior to entrance.  If your child has asthma which requires medication an Asthma Action Plan must be completed by a doctor prior to the first day of school.  If your child requires medication for a seizure disorder, a Seizure/Convulsion/Epilepsy Disorder Medication Administration Authorization Form must be completed by a doctor prior to the first day of school.  If your child has any other health concerns that require intervention while at school, an Individualized Treatment/Care Plan Checklist for Specialized Services must be completed by your doctor prior to the first day of school.

Form #2: Maryland Immunization Record – A copy of your child’s current immunization record from their pediatrician is required BEFORE the first day of class.  This is a state requirement. The doctor may provide you with a printed copy of your child’s record or use a Maryland Department of Health and Mental Hygiene Immunization Certificate form.

Form #3: Maryland Emergency Form – An Emergency Form must be completed by parents of every preschool child.  This form is used in case of emergency should the preschool need to contact parents.  It also provides the preschool with the names of all people authorized to pick up your child at the end of the day.

Form #4: Signed Guide to Regulated Care Acceptance – After reviewing the Maryland State Document Guide to Regulated Careplease complete and return a signed Guide to Regulated Care Acceptance Form. Only the acceptance form must be returned with your paperwork.
